Mountain Ridge Rehabilitation and Healthcare Center in Black Mountain, NC is seeking an experienced MDS Coordinator to manage the written plan of care for each resident, ensuring accuracy and regulatory compliance. You will collaborate with the MDS team to develop care plans, monitor status changes, and ensure documentation meets federal and state standards.

This role requires a current nursing license, CPR certification, and prior long-term care nursing experience.

Qualifications

• Nurse licensed in the current state with an active license.
• CPR certification.
• At least 1 year of nursing experience in long-term care.

Responsibilities

• Stay current with federal and state regulations and professional standards.
• Assist with development of comprehensive care plans for residents in coordination with the MDS.
• Coordinate, manage, and monitor the written plan of care for each resident.
• Assist nursing management with the coordination, management, and review of nurse's notes.
• Monitor resident status changes to ensure timely nursing or clinical involvement.
• Assure MDS and supporting documentation are accurate and meet regulatory and auditor requirements.
• Perform regular audits of documentation to ensure accuracy.
• Assist with the discharge process.
• Complete necessary forms and reports.
• Maintain strict confidentiality regarding sensitive health information; report hazards.

Job Summary

As an MDS Coordinator, you will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved. You will work closely with the MDS team to develop comprehensive care plans, monitor resident status changes, and ensure accurate documentation. Your exceptional organizational skills and attention to detail will be essential in ensuring compliance with federal and state regulations, as well as professional standards.

Key Responsibilities

• Keep abreast of current federal and state regulations, as well as professional standards


• Assist with the development of comprehensive care plans for residents in coordination with the MDS


• Coordinate, manage, and monitor the written plan of care for each resident


• Assist nursing management with the coordination, management, and review of nurse's notes


• Monitor resident status changes to ensure timely nursing or clinical involvement


• Assure MDS and support documentation are accurate and meet regulatory and auditor requirements


• Perform regular audits of documentation to ensure accuracy


• Assist with the discharge process


• Complete necessary forms and reports

• Report hazardous conditions, damaged equipment, and supply issues to appropriate persons

Requirements

• Current State License as a Nurse


• C.P.R. Certified


• One year experience as a Nurse in a long-term care setting (preferred)
