Blueberry Hill Rehabilitation and Healthcare Center is seeking an MDS Coordinator to lead timely, accurate MDS assessments and ensure regulatory compliance. This role supports resident care planning, reimbursement, and quality outcomes within a 132-bed Sub-Acute and Long-Term Care setting.
As a critical member of the team, you will supervise data entry, verify MDS submissions, and provide reporting to the DON and leadership. Active RN license and 3+ years of LTC experience are required.
