Job description

An MDS nurse, also known as a Minimum Data Set nurse or Nurse Assessment Coordinator, isa healthcare professional primarily responsible for conducting comprehensive assessments of residents in long-term care facilities, collecting data on their health needs using a standardized "Minimum Data Set" form, and utilizing that information to develop personalized care plans, ensuring compliance with federal regulations and optimizing Medicare/Medicaid reimbursement for the facility;they often collaborate with other healthcare team members to coordinate resident care and advocate for their well-being.

Key responsibilities of an M.D.S nurse:

  • Conducting resident assessments:

    Performing thorough evaluations of residents' physical, cognitive, and functional abilities upon admission and at scheduled intervals using the MDS assessment tool.

  • Data collection and documentation:

    Accurately recording resident information on the MDS form, including medical history, functional status, cognitive abilities, and care needs.

  • Care plan development:

    Utilizing MDS data to create individualized care plans for each resident, outlining specific goals and interventions to address their needs.

  • Interdisciplinary team collaboration:
