The ideal candidate holds a current RN or LPN license, with experience in MDS and familiarity with OBRA guidelines. Strong organizational and instructional skills are essential for success in this role at Apple Rehab.

Qualifications

• Must hold a current RN or LPN license and be in good standing.
• Experience with MDS (minimum data set) preferred; knowledge of OBRA and Medicare guidelines is required.
• Ability to manage multiple tasks with strong organizational skills.

Responsibilities

• Lead the MDS department and coordinate discharge planning.
• Assess and coordinate care for short-term residents and ensure Medicare compliance.
• Develop comprehensive care plans with interdisciplinary input and monitor plan adherence.
