Job summary

Ascent Healthcare Management is seeking an experienced MDS Coordinator to manage the Resident Assessment Instrument process in Western North Carolina facilities (Asheville, Hendersonville, Brevard, Salisbury and Rutherfordton).

The role emphasizes timely MDS assessments, care planning coordination, and Medicare/Medicaid reimbursement support, collaborating with nursing, therapy, social services, dietary, activities, and billing teams.

Qualifications

• Active RN or LPN license in good standing.
• Experience in long-term care or skilled nursing facility.
• Strong understanding of resident assessment, care planning, and clinical documentation.
• Ability to manage deadlines, prioritize work, and maintain accuracy.
• Strong communication and organizational skills.

Responsibilities

• Coordinate and complete accurate MDS assessments in accordance with federal and state requirements.
• Ensure MDS assessments are completed, signed, transmitted, and accepted timely.
• Maintain the MDS schedule (OBRA, PPS, significant change, quarterly, annual, admissions, discharges).
• Monitor assessment reference dates, look-back periods, completion dates, and transmission deadlines.
• Review validation reports and correct any rejected submissions promptly.
• Ensure Care Area Assessments are completed to support care planning.
