Job summary

Ayden Healthcare Oregon seeks an experienced MDS/Resident Assessment Assistant (RN or LPN) to coordinate and manage resident assessments in our Ohio facility, reporting to the Director of Nursing. The role emphasizes OBRA/MDS compliance, data collection (CASPER reports), care plan updates, and collaboration with IDT and therapies.

Ohio licensure is required. You will participate in pre-certifications, re-certifications, care conferences, and surveys to maintain regulatory standards and quality

Qualifications

• Experience coordinating resident assessments and data collection.
• Knowledge of OBRA, MDS requirements, and licensure regulations.
• Ability to analyze data and support IDT.

Responsibilities

• Coordinate data collection to complete the resident assessment instrument accurately and timely.
• Implement care plans with IDT approach and update OBRA assessments and daily changes.
• Evaluate resident needs and level of care per licensure and MDS requirements.
• Participate in managed care pre-certifications and re-certifications as needed.
• Attend care conferences and QAPI meetings as scheduled.
• Obtain CASPER reports and analyze data for the IDT.
• Ensure timely completion of assessments across departments and educate on compliance.
• Develop and review nursing policies and procedures.
• Assist with surveys and maintain 671/817 compliance.
• Coordinate with therapies for OBRA/PPS/OMRA assessments and data collection.
