Job Title: MDS Assistant / Restorative Nurse
Location: Aperion Care
Summary:
The Restorative Nurse is responsible for developing, implementing, monitoring, and supervising the restorative nursing program. Acts as a liaison between therapy and nursing, promoting a restorative philosophy across departments.
Essential Duties and Responsibilities:
- Develop and implement nursing care policies aligning with facility goals.
- Assess quality of care with nursing leadership, identifying improvements.
- Conduct daily rounds to evaluate patient care and staff performance.
- Ensure compliance with legal and regulatory standards through teaching and supervision.
- Perform rehabilitation assessments for all new admissions and periodically for existing patients.
- Oversee functional evaluations and collaborate with nurses on restorative care plans.
- Ensure restorative measures are documented on care plans.
- Coordinate with therapy departments and ensure recommendations are followed through.
- Review documentation for residents receiving restorative services.
- Manage incontinent care programs.
- Participate in care planning, QAPI meetings, and provide feedback on restorative services.
- Coordinate staff orientation and ongoing education on restorative nursing.
- Engage in fall prevention and restraint reduction programs.
- Setup documentation flow sheets for restorative measures.
- Supervise restorative nursing staff and serve as a liaison with therapy teams.
- Participate in care planning and Medicare (MDS) coordination as needed.
- Provide high-quality, compassionate care to residents and families.
- Perform other duties as assigned.
Qualifications:
- 3-5 years of restorative nursing experience required.
- Current Restorative Nurse certification preferred, must be registered with State Board of Nursing.
- Current CPR certification required.
Physical Demands:
- Ability to move intermittently, cope with stress, and work effectively with residents and staff.
- Ability to lift/move at least 25 pounds and assist in emergency evacuations.
