Journey is seeking an MDS Assistant to join our local Care Team in West Virginia. You will participate in the Resident Assessment Process, complete MDS items and CAAs, and help develop individualized resident Care Plans to reflect needs and projected outcomes.

This on-site role works with nursing staff to ensure accurate assessments, deliver compliant care, and support positive resident outcomes while adhering to federal and state regulations; competitive benefits included.

Qualifications

• A professionally trained Licensed Nurse with a nursing certificate/diploma or degree.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsabilités

• Participates in the Resident Assessment Process and completion of MDS items and CAAs.
• Assists in developing individualized resident Care Plans and identifies needs and projected outcomes per regulations.
