Job summary

Freedom Village Bradenton is seeking a MDS Assistant with active CNA certification to support the MDS/Care Plan office in a skilled nursing facility. You will coordinate care plans, assist with documentation, and serve as a liaison among nursing, therapy, and the business office.

Ideal candidates have CNA certification, experience in long-term care, and proficiency with EHR systems and Microsoft Office. This onsite role emphasizes accuracy, confidentiality, and collaborative care planning.

Qualifications

• Active, unencumbered CNA certification in good standing.
• Experience in skilled nursing or long-term care preferred.
• Familiarity with MDS/RAI process, care planning, and EHR systems such as Point-Click-Care or Matrixcare.
• Strong clerical, scheduling, and data-entry skills.
• Excellent organizational abilities and attention to detail.
• HIPAA confidentiality compliance.

Responsibilities
