Job summary

Scenic Wellness Nursing & Rehabilitation in Herculaneum, MO is seeking a dedicated MDS Assistant to support the Director of Nursing and the RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines.

You will assist with the RAI process from the MDS through the interdisciplinary care plan, support timely MDS submissions, and help maintain accurate records in PointClickCare to ensure high-quality resident care.

Qualifications

• Licensed in the state of practice is required.
• Minimum of 2 years’ nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of OBRA regulations and Minimum Data Set (MDS) processes.
• Understanding of Case-Mix, Medicare PPS, and Medicaid reimbursement.

Responsibilities

• Assist adherence to Federal and State regulations and certification.
• Participate in regulatory surveys and correction of deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Assist in completing the RAI process from MDS to care plan.
• Initiate and monitor RAI tracking and Medicaid forms in PointClickCare.
• Follow up with staff to ensure documentation standards.
• Complete patient assessments and data collection as needed.
• Determine Assessment Reference Date reflecting patient care needs.
• Submit MDSs to the State with follow-up on validation errors.
• Maintain validation records in an organized manner.

MDS Assistant

Principal Responsibilities

• Assist the center in assuring adherence to Federal and State regulations and certification
• Actively participate in the regulatory or certification survey process and the correction of deficiencies
• Report trends from completed audits to the Quality Assurance Committee
• Assist in the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care
• Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the PointClickCare system
• Follow up with staff when necessary to assure compliance to standards of documentation
• Complete patient assessments, data collection, and interviews with staff as necessary to assure good standard of practice
• Facilitate accurate determination of the Assessment Reference Date that accurately reflects the patient's care needs
• Ensure timely submission of the MDSs to the State with proper follow-up on validation errors
• Maintain validation records from the submission process in a systematic and orderly fashion

Qualifications

• Graduate of an approved Registered Nurse/Licensed Vocational Nurse program and licensed in the state of practice required
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement
• Through understanding of the Quality Indicator process
• Knowledge of OBRA regulations and Minimum Data Set
• Knowledge of the care planning process
