Job summary

Yakima Neighborhood Health Services is seeking a team member for maternity case management to support pregnant and postpartum clients with resources and referrals to promote self-sufficiency.

The role requires a Bachelor’s or higher in a social service field (or eligible substitution) and bilingual English/Spanish abilities. A background check will be completed as part of the process and the team collaborates to maintain quality care.

Qualifications

• Bachelor’s or higher degree in a social service-related field is required.
• Associate degree in behavioral or health sciences may substitute for experience requirements.
• Must have good attention to detail; strong written and verbal communication skills.

Responsibilities

• Provide resources and referrals to pregnant/postpartum clients to promote self-sufficiency.
• Conduct maternity case management intake/assessment to identify client needs.
• Attend regular case conferences with YNHS MSS team to address client needs.
• Assist in developing quality assurance standards for maternity case management.
• Develop and maintain linkages with community agencies, prenatal providers, and Maternity Support team members.
• Participate in staff in-service meetings.
• Engage in quality assurance and peer review as requested.
• Maintain organized, confidential work area; represent YNHS in local meetings.
• Perform related work and other tasks as requested.

• Bachelor’s or higher degree in a social service-related field and at least on year of full-time experience working in a social service field. Associate degree in behavioral or health sciences or related field and two years closely related work experience may be substituted.

• Candidate must not have been sanctioned or excluded from participation infederal or state health care programs by a federal or state law enforcement, regulatory, or licensing agency.

Day to Day:
