Job summary

VHC Health is seeking a Perinatal Care Coordinator to coordinate comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients. This role acts as a liaison among patients, families, physicians, nursing staff, case management, and community resources to ensure safe, timely discharges.

The position requires a Bachelor’s degree in Nursing, at least three years of maternal-child health experience, RN licensure in Virginia or compact state

Qualifications

• Bachelor's degree in Nursing is required.
• Minimum of three years of experience in maternal-child health, obstetrics, neonatal care, pediatrics, or related healthcare setting.
• Licensed as a Registered Nurse by the Virginia Department of Health Professions required or, Compact State Licensure in Nursing required.
• BLS certification required.

Responsibilities

• Coordinate comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients.
• Serve as liaison between patients, families, physicians, nursing staff, case management, and community resources to ensure safe, timely, and well-coordinated care and discharges.
• Provide education, connect families with hospital and community resources, facilitate referrals, and address barriers to care to improve patient outcomes.
• Promote continuity of care following hospitalization.
