Job summary

VHC Health is seeking a Perinatal Care Coordinator to coordinate comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients. This role acts as a liaison among patients, families, physicians, nursing staff, case management, and community resources to ensure safe, timely discharges.

The position requires a Bachelor’s degree in Nursing, at least three years of maternal-child health experience, RN licensure in Virginia or compact state

Qualifications

• Bachelor's degree in Nursing is required.
• Minimum of three years of experience in maternal-child health, obstetrics, neonatal care, pediatrics, or related healthcare setting.
• Licensed as a Registered Nurse by the Virginia Department of Health Professions required or, Compact State Licensure in Nursing required.
• BLS certification required.

• Coordinate comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients.

The Women & Infant Perinatal Care Coordinator is responsible for coordinating comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients. This role serves as a liaison between patients, families, physicians, nursing staff, case management, and community resources to ensure safe, timely, and well-coordinated care and discharges. The coordinator provides education, connects families with hospital and community resources, facilitates referrals, and addresses barriers to care to improve patient outcomes, reduce readmissions, and promote continuity of care following hospitalization

Education:

Bachelor's degree in Nursing is required.

Experience:

Minimum of three years of experience in maternal-child health, obstetrics, neonatal care, pediatrics, or related healthcare setting.

Certification/Licensure:

Licensed as a Registered Nurse by the Virginia Department of Health Professions required or,

Compact State Licensure in Nursing required

BLS certification required
