Competenze

• Bachelor’s degree from an accredited college or university in social work, social sciences, or a health-related field.
• Minimum of three years of verifiable professional experience in social work, health education, nursing or counseling services.
• Fluent in Creole and English (spoken and written).
• At least one year of verifiable experience in home visits with clients.
• At least one year of verifiable experience conducting interviews or assessments in health or social services.

Mansioni

• Provide unique, specialized case management to pregnant women and at-risk families; coordinate service delivery.
• Conduct assessments of medical and social needs; provide referrals, counseling and guidance.
• Deliver care coordination services via home, office, or community settings to BBB program participants.
• Develop and monitor care plans; document client contacts and service plans; ensure timeliness and quality of care.
