Job summary

Maternal and Family Health Services across Bradford and Tioga counties seeks a Maternal Health Advocate to connect MFHS clients with MFHS programs and community partners, deliver education, and support for prenatal, perinatal, and postpartum care. This position is instrumental in promoting service integration and navigating available perinatal services.

The Advocate builds relationships, assists with insurance and appointments, coordinates care, and promotes access to a wide range of MFHS

Qualifications

• High School Diploma or GED required; Bachelor's in public health preferred.
• Community Health Worker certification preferred but must be obtained within one year of hire.
• One year of experience in a community setting with diverse families.
• Knowledge of psychosocial, economic, and cultural determinants to health.
• Excellent communication and interpersonal skills.
• Bilingual English/Spanish preferred.
• PA driver's license or access to reliable transportation for outreach.
• Understanding of medical office processes and social service delivery.

Responsibilities

• Care Navigation: connect clients to MFHS programs and community resources; assist with referrals and eligibility; help with insurance applications and transportation arrangements.
• Preparing Clients for Appointments: provide reminders and required documentation; manage pre-certifications and follow-ups.
• Promote Service Integration: coordinate care across MFHS programs and document referrals.
• Engage with Community Partners: build partnerships across health, education, housing, and social services.
• Assist with Outreach: participate in events to promote MFHS and referrals.
• Administrative Support: front desk help, surveys, translation, and education resources.
• Promote and support MFHS goals and assist with car seat installation and distribution activities.
