Job summary

The Ohio State University Wexner Medical Center seeks an RN Case Manager for the Moms2B program. You will serve as a nurse educator, developing and teaching maternal and infant health curricula across multiple sites to support pregnant women and their babies in reducing mortality risks.

You will advocate for patients, coordinate care, and collaborate with a multidisciplinary team. Ohio nursing licensure and BCLS certification are required; 2+ years RN experience and case management exposure are

Qualifications

Bachelor's degree in Nursing required.
Master's degree desired.
Current Ohio licensure, and current BCLS certification required.
Minimum of 2 years of experience as an RN preferred.
Experience in case management desired.

Responsibilities

Develop, teach educational programs for the maternal and infant health curriculum in Moms2B.
Advocate to ensure access to appropriate care and services; coordinate patient care.
Navigator to identify and remove barriers to care within the healthcare continuum.
Collaborate with multidisciplinary team to ensure a comprehensive plan of care.
Educate patients and caregivers about diagnoses, treatment, and symptoms.
Manage a sub-population of patients to optimize resource use.
Participate in fundraising and donor awareness activities as necessary.
