Job summary

Department of Health in Clearwater Center, part of the State of Florida, seeks a Human Services Counselor I to establish trusting relationships with diverse families and provide case management for pregnant women and infants in the Healthy Start program.

You will conduct face-to-face visits, offer parenting and breastfeeding education, and document client information in the Well Family System, coordinating referrals to community resources.

Qualifications

• Experience in Counseling techniques and strategies.
• Experience working with pregnant women and children.
• Problem solving skills related to advancing client self-sufficiency.
• Ability to work well with others as a team.
• Computer skills: competency and experience.
• Experience in interviewing and assessing to determine the needs of the client.
• Knowledge of community resources.

Responsibilities

• Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
• Provide Case Management to pregnant women and infants referred to the Healthy Start Program.
• Provide face to face contact with clients either in the OB provider’s office or in the client’s residence.
• Provide Parenting Education one on one with clients.
• Provide Breastfeeding Education and Tobacco Cessation Education to individual clients.
• Document all client contact information in the web-based computer application known as the Well Family System
• Provide referrals to community resources to supplement Healthy Start services.
