Job summary

The State of Florida Department of Health seeks an OPS Family Support Worker to engage pregnant women and families with young children, assess risks, and link to home visiting services. You will conduct intakes, educate families on resources, and maintain HIPAA-compliant records in the Well Family System. Travel to service area may be required.

Strong communication and multitasking are essential. Preferred bilingual capability and at least 1 year of social service experience; proficiency with MS

Qualifications

• At least 1 year of social service-type work experience and proficiency with Microsoft Office.
• Preferred - High school education; bilingual/multilingual in English and other language(s). Two years of social service-type work experience.

Responsibilities

• Contact pregnant women, interconception women and families to assess risk factors.
• Provide information and linkage to home visiting programs and resources.
• Conduct initial client intakes and follow up on referrals.
• Enter participant data into the Well Family System and maintain confidentiality per HIPAA.
