The role requires home visits, coordination with providers, and ongoing QA efforts to improve service delivery.

Core duties include developing care plans, training family support networks, maintaining accurate records, and collaborating with funding sources to ensure resources.

Qualifications

• Bachelor’s degree in a relevant field is required.
• Minimum 3 years of relevant experience required.
• Bilingual, Creole or Spanish preferred.
• Strong communication and teamwork skills.

Responsibilities

• Provide maternal child health services to pregnant women and infants.
• Conduct home visits and coordinate with community providers.
• Develop individualized care plans and wraparound services including parenting education and breastfeeding support.
