Job summary

Renown Health in Reno, NV seeks a Case Manager (Maternal Child) to coordinate care across the health continuum, support discharge planning, and reduce avoidable days. This role leads the interdisciplinary team and ensures timely reviews of medical records and utilization management.

Candidates will have RN licensure in Nevada, with BSN preferred and CCM/ACM certification preferred; strong documentation and communication skills required; experience in case management and UR/QA a plus.

Qualifications

• Must hold Nevada RN licensure; English proficient.
• BSN in Nursing preferred.
• CCM or ACM certification preferred.

Responsibilities

• Assess and coordinate patient care across the health continuum.
• Lead interdisciplinary team to develop and update discharge plans.
• Review medical records to ensure timely and appropriate services.
• Document progress and communicate with payers and care teams.
