Sun River Health provides comprehensive primary, preventative, and behavioral health services to all, regardless of insurance status or ability to pay, especially serving underserved and vulnerable populations. We are a Federally Qualified, Non-Profit Health Center serving communities across New York State, including New York City, Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester. We are seeking a full-time Registered Nurse/ MAT Care Manager for our Staten Island location.

Position Summary

The Registered Nurse/ MAT Care Manager is part of a multidisciplinary team that uses the nursing process to assess, plan, implement, and evaluate patient care and resource use. Responsibilities include monitoring quality of care, ensuring infection control and risk management, advocating for patients, providing care and crisis management, and participating in harm reduction, opioid overdose prevention education, and staff training.

Essential Duties and Responsibilities

  • Serve as the primary contact and point person for the site.
  • Complete MAT intakes and care plans both by phone and in person.
  • Develop recovery support plans and document all encounters.
  • Coordinate care with external agencies through consent forms.
  • Assist with discharge paperwork, including drafting and scanning.
  • Triaging issues, providing basic nursing care during visits, and pre-visit planning.
  • Perform I-STOP & PNP procedures, opioid overdose prevention, prior authorizations, labs review, and urine drug screens, including pregnancy tests for women of childbearing age.
  • Manage case management tasks, referrals, and patient education on stigma and overdose prevention.
  • Liaise with pharmacy, schedule and reschedule appointments, and monitor patient panels.
  • Participate in program initiatives and case conferencing.

Essential Job Qualifications

  • Bachelor’s Degree and at least one year of experience in an acute care setting.
  • New York State RN license and BLS/CPR certification required.

Job Type: Full-Time
Pay Range: $45.00 - $53.00 per hour
