$38.00/hr - $41.65/hr

Position Summary

The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to meet an individual’s benefit plan and health needs through communication and available resources to promote optimal, cost-effective outcomes.

Duties
  • Complete at least 10 NF member assessments weekly
  • Support the Health Plan Rebalancing Initiative by assessing, identifying, screening, and transitioning NH members into the community
  • Follow up on referrals and visit current NH members in-person at least twice a week to complete rebalancing events and assessments
  • Conduct telephonic or in-person home assessments prior to discharge to identify environmental supports needed for transition
  • Perform Significant Change Visits within 5 days of transition, coordinate services, establish Plan of Care, and document actions
  • Coordinate with facility’s Business Office weekly to follow-up on member census and discharge planning
  • Collaborate with case managers to identify high-risk community members and implement interventions to prevent lapses or ensure safe transitions
  • Enhance healthcare value to increase member satisfaction and retention, and support membership growth
  • Participate in at least two community events annually
  • Build strong relationships contributing to member satisfaction and retention
Required Qualifications
  • Reside in Tampa area, able to travel to Hillsborough, Highlands, Polk, Hardee, and Manatee Counties, with up to 75% local travel
  • Minimum 2 years of clinical experience
  • Bi-Lingual Spanish/English
  • Willing and able to travel extensively within the region
