Job summary

SaVida Health is seeking a dedicated Registered Nurse for the MAT program in Vermont. The role focuses on comprehensive care management, care coordination, and patient advocacy to support individuals with substance use disorders.

Qualifications

• Must be currently licensed as a Registered Nurse in Vermont.
• Demonstrated ability to connect patients to area resources and advocate for them.
• High-visibility role requiring excellent interpersonal skills and community interfacing.
• Supports mission to improve lives of individuals with substance use disorder.
• Ability to work independently and as part of a team.
• Ability to plan and prioritize work with limited supervision.
• Knowledge of local social services, health, and community resources a plus.
• Ability to liaison with participants, monitors, employees and liability providers.
• One year of EHR experience; Athena EHR experience a plus.
• Computer literacy; typing skills; familiarity with EHR/EMRs.
• Ability to work with diverse populations; bilingual/bicultural preferred.
• Excellent verbal and written communication skills.
• Ability to present information in group settings.
• Time management skills.
• Ability to administer medical injections.
• Proficiency in clinical documentation (assessments, treatment planning, progress notes).

Responsibilities

• Provide comprehensive care management and care coordination for MAT patients.
• Coordinate transitions of care and referrals to community supports.
• Promote patient activation and self-management of health and substance use conditions.
• Participate in weekly staff meetings and supervision.
• Maintain documentation and ensure program compliance with policies.
• Collaborate with therapists and medical providers to meet patient needs.
• Build connections with community agencies and service providers.
• Advocate for patients and liaise with providers and insurers as needed.
