Job summary

SaVida Health is seeking a Spoke Nurse for the MAT program in Vermont. This full-time weekday role focuses on comprehensive care management for patients with substance use disorders.

The RN must be licensed in Vermont and skilled in care coordination, documentation, and collaboration with medical and behavioral health teams. Responsibilities include coordinating care, promoting health and self-management, and ensuring smooth transitions between treatment settings.

Qualifications

• Current licensed in Vermont required.
• Ability to connect patients to local resources and advocate for them.
• Excellent interpersonal and written communication skills.
• Experience with Electronic Health Records (Athena or similar).
• Bilingual/bicultural preferred.

Responsibilities

• Provide comprehensive care management and care coordination for patients in Medication Assisted Treatment.
• Coordinate referrals and transitions across care settings to reduce hospital readmissions.
• Promote health, self-management and patient activation.
• Document clinical information and treatment plans in the EMR.
• Collaborate with behavioral and medical teams to meet patient needs.
• Participate in staff meetings and quality improvement activities.
