Job summary

Liberation Programs is a leading behavioral health organization in Fairfield County offering treatment for substance use disorders and various supports since 1971. The Outreach Specialist/MAT Van Driver role sits within Community Outreach and Homeless Services to bridge harm reduction and addiction treatment through low-barrier, trauma-informed outreach.

The position is grant-funded for the term of the funds.

Qualifications

• Excellent communication skills.
• Understanding and experience with medication-assisted treatment, outpatient and residential treatment.
• Must have a valid driver's license and be comfortable driving a large van.

Responsibilities

• Daily monitoring of Cordata (Software Platform) for overdose referrals.
• Coordinate overdose response follow-up visits with Outreach Operations Manager / Senior Director of Harm Reduction.
• Provide community engagement through daily outreach to connect with at-risk populations.
• Follow up with individuals at risk of overdose to maintain positive therapeutic relationships and provide resources.
