Job summary

Liberation Programs is a leading behavioral health organization in Fairfield County, providing treatment, outreach, and housing services since 1971. The Outreach Specialist/MAT Van Driver position supports harm reduction and low-barrier access, delivering case management and connections to detox, treatment, housing and health services.

The role operates in the Community Outreach and Homeless Services department and is grant-funded for the term of the award.

Qualifications

• Minimum of one year working with people with SUD/SMI or equivalent lived experience.
• Excellent communication skills required.
• Understanding and experience with medication-assisted treatment, outpatient and residential treatment.
• Must have a valid driver's license and be comfortable driving a large van.
• Familiarity with local support services and recovery resources preferred.

Responsibilities

• Monitor Cordata for overdose referrals from partner agencies on a daily basis.
• Coordinate overdose response follow-up visits with Outreach Operations Manager / Senior Director of Harm Reduction (CLEAR Site Lead).
• Attend Deflection Academy Training.
• Engage the community through daily outreach to connect with at‑risk populations.
• Attend community events including Naloxone Training/Overdose Prevention.
