Liberation Programs is a leading behavioral health organization in Fairfield County, providing services for substance use disorders and related needs. The Outreach Specialist/MAT Van Driver role supports low-barrier, trauma-informed outreach and connects individuals to detox, treatment, housing and health resources.

The position is grant-funded for the term of the grant and involves daily on-site and field outreach, with emphasis on engaging at-risk populations and delivering practical

Qualifications

• Minimum one year working with people with SUD/SMI or equivalent lived experience.
• Excellent communication skills.
• Understanding and experience with MAT, outpatient and residential treatment.
• Valid driver’s license and insurance; comfortable driving a large van.
• Familiarity with local support services and recovery community preferred.

Responsibilities

• Monitor Cordata software for overdose referrals.
• Coordinate follow-up visits with Outreach Ops Manager / CLEAR Site Lead.
• Attend Deflection Academy Training.
• Provide community outreach to connect with at-risk populations.
• Attend and support community Naloxone/Overdose Prevention events.
• Follow up with individuals at risk of overdose to maintain rapport and provide resources.
• Maintain professional boundaries with empathy and non-judgmental approach.
• Participate in Hazardous Waste Collection at sites as required.
• Develop referrals to Detox/Treatment, Housing, Medical/Primary Care, and other services.
