Homeward Bound in Dallas seeks a fully licensed LCDC to join the outpatient Medication-Assisted Treatment (MAT) program. The role blends administrative and clinical duties and requires high attention to detail, self-direction, and trauma-informed care.

The position involves completing admissions, eligibility, treatment plans, and discharge documentation, coordinating with prescribers, and managing funding sources. It is a full-time, Mon-Fri role.

Qualificações

• Must hold a valid LCDC license in Texas.
• Experience in behavioral health and MAT settings preferred.
• Ability to work self-directed with strong attention to detail.

Responsabilidades

• Complete admission documentation including screening, assessment, and treatment plan for MAT clients.
• Complete initial financial eligibility form and update every 6 months.
• Perform treatment plan reviews every 30 days.
• Conduct individual counseling sessions regularly.
• Prepare discharge documentation after staffing discussions.
• Coordinate with prescribers and medical staff on clinical concerns.
• Navigate funding sources to coordinate individualized care.
• Provide continuity of care with internal and external programs.
• Assist with medication issues (pharmacy communication, pick up, discrepancies).
• Assist with urine drug screens and documentation.
• Collect MAT program data in CMBHS and Netsmart; maintain organizational records.
• Explain Subutex, Suboxone, and Sublocade to clients and staff.
