The position is full-time, Monday through Friday, and involves documentation, care coordination, and interfacing with prescribers while working with systems like CMBHS and Netsmart.

Qualifications

• Fully licensed LCDC
• Experience with MAT programs preferred
• Knowledge of CMBHS and Netsmart systems preferred

Responsabilités

• Complete admission documentation including screening, assessment, and treatment planning for MAT clients.
• Perform initial financial eligibility forms and re-evaluate every 6 months.
• Update and review treatment plans every 30 days.
• Conduct individual counseling sessions regularly.
• Prepare discharge documentation after staffing discussions and MAT completion or disengagement.
• Coordinate with prescribers and other medical professionals on clinical concerns.
• Coordinate funding sources to tailor individualized care plans.
• Ensure continuity of care with internal and external programs.
• Assist with medication-related communications with pharmacies and reconciliation.
• Assist with urine drug screens and documentation as needed.
• Support MAT data collection, surveys, and record-keeping across CMBHS and MAT documentation.
