Job summary

Mi Casa Hospice in San Antonio, TX is seeking a PRN Master Social Worker to join our patient-centered care team.

The MSW will provide emotional support, psychosocial assessment, counseling, crisis intervention, and bereavement support to patients and families, helping them navigate end-of-life decisions.

This PRN role offers flexible scheduling and the chance to work collaboratively with an interdisciplinary team to ensure dignity, comfort, and peace for our patients and their loved ones.

Qualifications

• Must hold MSW from accredited program.
• Texas LMSW licensure required.
• One year of hospice/palliative care preferred.

Responsibilities

Position Summary

We are seeking a PRN Master Social Worker (MSW) to join our hospice care team. The ideal candidate will provide emotional, psychosocial, and resource-based support to patients and families, helping them navigate end-of-life care with dignity and understanding.

Key Responsibilities

• Assess patients’ and families’ psychosocial, emotional, and support needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide counseling, crisis intervention, and grief/bereavement support.
• Assist families in accessing community resources and support services.
• Maintain accurate and timely documentation in accordance with state, federal, and agency guidelines.
• Participate in care conferences, team meetings, and patient/family education.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited program.
• Current Texas licensure as LMSW.
• At least one year of hospice or palliative care experience preferred.
• Strong communication, counseling, and advocacy skills.
• Ability to work independently and collaboratively in a team setting.
