Job summary

Devotion Hospice in Corpus Christi, TX is seeking a Master Social Worker (LMSW) to provide coordinated home-based care to patients of all ages, performing psychosocial assessments, counseling, and referrals.

You will assess within 5 days of hospice election, assist with advance directives, discharge planning, and family counseling, and collaborate with the IDG to optimize patient care while protecting privacy.

Qualifications

• Masters degree in Social Work from an accredited school.
• Licensed SW.
• One or more years of Hospice experience required.

Responsibilities

• Provide coordinated home-based care to patients of all ages, performing psychosocial assessments, analysis, counseling and referrals.

• Provide patient and family/caregiver counseling around end-of-life, death, and grief.
• Attend interdisciplinary team meetings, coordinate with the IDG to promote care coordination.
• Promote respect for patients' privacy and property.

Position Summary

Primary function is to provide coordinated care in the home to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family.

As a Social Worker , you will:

Assesses and completes the Comprehensive Assessment within 5 days of the patient’s election of hospice care to identify patient and family’s psycho social, financial, environmental and community needs as evidenced by POC documentation, clinical records, IDG meetings and community resource referrals.

Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.

Use effective interpersonal relations and communication skills to achieve desirable outcomes.

Provides patient and family/caregiver counseling around issues of end-of-life, death, and grief.

Attends interdisciplinary team meetings, participates in the patient care planning process and coordinating and collaborating with the IDG to promote coordination of patient care.

Provides an environment which promotes respect for patients and their privacy and property.
