Job summary

Klarus Home Health & Hospice/Jet Health Inc. seeks a PRN Licensed Medical Social Worker to support hospice patients in the San Antonio, TX region and surrounding areas. Primary duties include psychosocial assessments, counseling, and care coordination for patients across age groups.

The role emphasizes collaboration with the interdisciplinary team, documentation, and ongoing professional development while adhering to agency policies and QAPI initiatives.

Qualifications

• Master's or Doctoral degree from an accredited School of Social Work.
• Active social work licensure (LMSW/LICSW/LCSW) in the state.
• Minimum of one year MSW experience in a healthcare setting.
• CPR certification and valid driver's license.

Responsibilities

• Conduct ongoing clinical psychosocial assessments of hospice patients.
• Develop and evaluate the plan of care with patient, family and caregivers.
• Provide counseling and education as ordered by the physician and plan of care.
• Maintain accurate documentation of assessments, interventions, and communications.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Participate in QAPI program and in-service training.
• Stay updated on eligibility standards and maintain competency.
