Job summary

High Point & Affiliated Organizations in New Bedford, MA seeks a qualified clinical professional to provide crisis intervention and direct care to youth and families within a substance use and mental health program. Responsibilities include assessments, counseling, discharge planning, and collaborative care with families and external providers.

You will participate in meetings, maintain medical records, ensure trauma-informed practice, complete required trainings, and document time and service

Responsibilities

• Crisis intervention; Maintain assigned caseload; Providing direct care to patients/residents including assessments and individual counseling.
• Discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion.
• Completion of bio-psychosocial assessments; Making referrals when appropriate; Responsible for each assigned patients/residents medical records.
• All records are to be professional, complete and thorough with entries made in a timely manner.
• Responsible connecting patients/residents with aftercare services prior to discharge and aiding the resident and family access services when appropriate.
• Communicating/involving family in treatment at patients/residents request; Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor/Director.
• Responsible for continued education in the field of substance use, mental health and related courses.
• Always utilize standards precautions; Demonstrates behaviors that recognize the rights of patients as defined by the patients rights.
• Completion of all CBHI New Hire Trainings upon hire (IHT); CANS Certified at hire and as recertification is required.
• Possesses an understanding of trauma informed care and cultural competence and attends all additional trainings as required/assigned.
• Ability to run psycho educational and social support groups as needed.
• Knowledge of area resources necessary to meet treatment plan goals of youth; Ability to network and develop professional working relationships with external area providers.
• Place referrals for additional services for youth and families throughout treatment and prior to discharge, as necessary; Ongoing regular communication and collaboration with other internal and external providers.
• Completion of all required documentation at the start of services and updates as required by performance specifications; Ongoing monitoring and compliance with required documentation for services.
• Ability to accurately document time spent with clients, guardians, and collaterals including interventions utilized and responses to these interventions.
• Provide clinical services, both in person and through telehealth platform; Tracking service authorizations dates/units and providing updated information for continued stay reviews, as necessary for the services provided.
• Attend all required staff and client meetings, or additional meetings assigned by supervisor; Attend weekly clinical supervision, tracking all direct client contact and supervision hours toward licensure.
• Comply with all necessary medical necessity criteria and performance specifications, dependent on service provided.
