Qualifications

  • Minimum two years of direct psychotherapy with adults, children, adolescents, and families.
  • Experience in settings with both mental health and substance use treatment preferred.
  • Knowledge of NYS CCBHC model and whole-person care principles.
  • Familiarity with OMH Part 599 and OASAS Part 822 regulations.
  • Proficiency with computer systems and Electronic Health Records (EHR).
  • Ability to communicate effectively across all levels of the organization.
  • Bilingual Spanish/English is a plus.

Responsibilities

  • Conduct comprehensive psychosocial assessments for adults and/or children and evidence-based screenings for substance use and mental health.
  • Manage a caseload of approximately 30 to 32 patients.
  • Develop and monitor integrated, person-centered treatment plans with the multidisciplinary team.
  • Collaborate with care managers, peer specialists, and other providers to coordinate care.
  • Maintain accurate documentation in the EHR for all services rendered.
  • Provide crisis intervention as needed.
  • Attend weekly supervision and participate in team meetings.

Job description

The Behavioral Health Clinician provides direct clinical services as a key member of an integrated care team. The clinician's role is to deliver comprehensive, person-centered care that addresses both mental health and substance use needs, consistent with the NYS Certified Community Behavioral Health Clinic (CCBHC) model. This position requires a flexible and collaborative professional who is dedicated to providing high-quality, whole-person care and is capable of providing a broad range of services based on patient need.

Under supervision, the clinician performs functions related to counseling, diagnostics, treatment, treatment planning, rehabilitation, assessment, and other supportive services for patients. The clinician is an integral part of a multidisciplinary team, collaborating with peers, care managers, and other professionals to ensure continuity of care.

  • Qualified health professionals (LCSW, LMSW, LMHC, LMFT, or Licensed Psychologist) are preferred. Licensed eligible (Psychologist, MHC, MFT) professionals are eligible.
  • Previous clinical experience (minimum two years) providing direct psychotherapy services to adults, children, adolescents, and families.
  • Demonstrated experience in a setting that provides both mental health and substance use treatment is highly preferred.
  • Knowledge of and commitment to the NYS Certified Community Behavioral Health Clinic (CCBHC) model and its core principles, including whole-person care and team-based delivery.
  • Familiarity with OMH Part 599 and OASAS Part 822 regulations.
  • Proficiency with computer, information systems, and Electronic Health Records.
  • Ability to communicate with all levels across the organization effectively.
  • Comfort working in a community-based setting with a diverse patient population.
  • Excellent organizational, verbal communication, and writing skills.
  • Bilingual (Spanish/English) is a plus.
Responsibilities
  • Conduct comprehensive psychosocial assessments for adults and/or children and evidence-based screenings for substance use and mental health. Under supervision, the clinician performance functions related to assessment, diagnosis, treatment planning, treatment, counseling, rehabilitation, and other support services for patients.
  • Manage a caseload of approximately 30 to 32 patients, addressing a wide range of mental health and/or substance use disorders.
  • Develop, and continuously monitor, integrated, person-centered treatment plans in collaboration with patients and members of the multidisciplinary team.
  • Actively participate as a core member of an integrated care team, collaborating with care managers, peer specialists, and other medical providers to coordinate care and ensure a seamless patient experience.
  • Make decisions about, and maintain required records of, appropriate Level of Care using patient criteria tools (e.g.,NYS-LOCADTR) as necessary for patients receiving substance use services.
  • Monitor Medication Management for patients under the guidance of a waivered prescriber with close clinical follow-up.
  • Conduct outreach for patient re-engagement efforts when patients miss scheduled clinical appointments.
  • Maintain accurate and contemporaneous documentation in the Electronic Health Record (EHR) for all services rendered, including progress notes, treatment plans, and discharge plans.
  • Provide crisis intervention for patients as needed.
  • Attend weekly individual and group supervision and contribute to clinical case conferences and team meetings.
  • Collaborate with the Quality Assurance and Admissions Departments to ensure all patient documentation is in compliance with regulations.
