Job summary

Equality Health is seeking a Provider Performance Advisor for the Louisiana market. This field-based role supports practices across markets, driving performance improvements, quality outcomes, and adoption of EH VBC strategies.

You will collaborate with physicians and leadership to enable value-based care initiatives and ensure sustainable change. The role requires extensive travel, strong stakeholder engagement, and experience with HEDIS/STARs, EHR systems, and population health workflows.

Qualifications

• Bachelor's degree in business, public health or related field, or equivalent experience.
• Willing to travel up to ~75% across markets.
• Minimum 3 years in healthcare industry, preferably in provider/practice management or revenue cycle roles.
• Experience with HEDIS/STARs, coding and billing.
• Experience with EHR/EMR systems.
• Ability to drive change and engage providers and staff.
• Quality certifications preferred (e.g., CPHQ).

Responsibilities

• Enable and enhance practice performance in the EH VBC Network.
• Support assigned practices in adopting the EH VBC program areas: access, preventive care, chronic condition management, and appropriate coding/billing.
• Align practice groups with network quality performance goals.
• Set weekly and monthly operational goals to achieve annual targets.
• Evaluate practice behavior change and quality performance against goals.
• Monitor and report progress to EH and practice leadership; escalate as needed.
• Identify barriers and provide actionable recommendations to surmount them.
• Drive sustainable operational and behavioral change that improves outcomes and VBC success.
• Train practices in markets with feedback loops to PAM.
• Represent EH in markets, building provider engagement and partnerships.
