Job summary

Equality Health seeks a Provider Performance Advisor to support market-based practices in Tennessee or Virginia. The role analyzes population health data, measures performance, and partners with physicians and leadership to drive value-based care initiatives.

You will train and support practices across markets, align goals, monitor progress, and ensure successful adoption of EH VBC strategies, with travel up to 75%. Strong collaboration with practice leadership is essential.

Qualifications

Bachelor’s degree in business, public health or related field.
Willing to travel up to approximately 75% across markets to support provider practices.
Minimum of 3 years of experience in the healthcare industry, preferably in provider practice management or quality roles.
Experience with HEDIS, STARs and medical coding & billing/revenue cycle.
Experience with Electronic Health/Medical Records (EHR/EMR).

Responsibilities

Enable and enhance practice performance in the EH VBC Network.
Support assigned practice teams implementing the EH VBC program across markets.
Align practice groups on network quality performance goals.
Establish weekly/monthly operational goals to achieve annual performance targets.
Measure and monitor practice population health management and report results to EH leadership.
Identify barriers and provide recommendations to overcome them.
Drive sustainable change to improve quality outcomes and patient care within VBC.
Travel to markets up to 75% to support provider practices.
