Monogram Market Physician Executive (MPE) are mission-driven physician leaders dedicated to improving the well-being, quality of life, and health outcomes for our patients. The MPE will lead our in-home multi-specialty polychronic care model in an assigned market. Each market comprises 5-10 practices led by local advanced practice providers (APP), registered nurses (RN), licensed clinical social workers (LCSW), and pharmacists (PharmD). The MPE collaborates with Monogram Health’s Multi-Specialty Platform to leverage employed specialists for in-home specialty care. Monogram deploys a risk-based model to ensure health equity and equality, utilizing proprietary AI algorithms to predict the appropriate level of care.

The MPE's primary goal is to deliver exceptional outcomes through disease detection, evidence-based clinical pathways, and disease treatment. Reporting to the Region President, the MPE is a key clinical leader contributing to the development and oversight of clinical strategies, policies, programs, processes, protocols, guidelines, and operations to improve patient health outcomes within the market. The MPE oversees daily clinical and business operations, including direct patient care, care management services, social worker support, and pharmacy services. The role involves reviewing and approving care plans, collaborating with community physicians, and participating in peer-to-peer and direct patient care decision-making.

Roles and Responsibilities
  1. Clinical Guidelines Execution - Ensure adherence to clinical pathways, review and approve care plans, document clinical activities, and co-sign encounters.
  2. Clinical and Operations Performance - Accountable for reducing costs, improving clinical outcomes, overseeing daily operations, and assessing market performance.
  3. Patient Care and Treatment - Provide direct and indirect patient care, engage with patients, order labs and referrals, and close care gaps.
  4. Population Health Management (PHM) - Guide strategies for high-risk patients, reduce hospital readmissions, and improve visit quality.
  5. Miscellaneous - Participate in on-call activities, with a minimum of 7 days per quarter, and provide coverage as needed.
Position Requirements
  • Remote within the designated market with occasional in-home visits and travel.
  • Experience applying evidence-based clinical criteria, especially in renal care and geriatrics.
  • Strong management and communication skills.
  • Active, unrestricted medical license in each state within the market.
  • Experience with Medicare Advantage, Medicaid, NCQA, HEDIS, risk adjustment, and quality improvement.
  • MD or DO degree from an accredited medical school; BC or BE in an approved specialty such as Nephrology, Internal Medicine, or Geriatrics.
