Market Physician Executive (MPE) - Birmingham, AL

Position Overview: The Monogram Market Physician Executive (MPE) is a mission-driven physician leader dedicated to improving patient well-being, quality of life, and health outcomes. The MPE will lead our in-home multi-specialty care model within an assigned market, collaborating with local practices, advanced practice providers, nurses, social workers, and pharmacists. The role involves leveraging AI algorithms to predict care needs, focusing on health equity and clinical excellence aligned with the quadruple AIM: patient experience, population health, provider satisfaction, and cost reduction.

Key Responsibilities:

  • Clinical Guidelines Execution: Ensure adherence to evidence-based pathways, review and approve care plans, and document clinical activities.
  • Performance and Quality Improvement: Oversee clinical outcomes, manage costs, lead high-risk reviews, supervise clinical staff, and assess market performance.
  • Patient Care and Treatment: Provide direct and indirect patient care, engage with patients, and collaborate with community providers.
  • Population Health Management: Guide strategies for high-risk patient management, reduce hospital readmissions, and analyze utilization trends.
  • Additional Duties: Participate in on-call activities, provide coverage during absences, and occasionally travel for in-home visits.

Position Requirements:

  • Remote work within the designated market with occasional travel.
  • Experience applying evidence-based clinical criteria, especially in renal care and geriatrics.
  • Strong management and communication skills.
  • Active, unrestricted medical license in the state(s) within the market.
  • MD or DO from an accredited school, BC or BE in relevant specialties such as Nephrology, Internal Medicine, Geriatrics, etc.
