Job description

Monogram Market Physician Executive (MPE) are mission-driven physician leaders dedicated to improving patient well-being, quality of life, and health outcomes. The MPE will lead our in-home multi-specialty care model in an assigned market, collaborating with local healthcare providers and leveraging Monogram Health’s platform and AI algorithms to deliver in-home specialty care. The role focuses on enhancing patient experience, population health, provider satisfaction, and cost management, aiming to achieve exceptional outcomes through evidence-based clinical pathways.

Reporting to the Region President, the MPE contributes to developing clinical strategies, overseeing daily operations, and managing clinical and business activities within the market. Responsibilities include reviewing and approving care plans, collaborating with community providers, and ensuring adherence to clinical guidelines.

Roles and Responsibilities

Clinical Guidelines Execution – 25%

  • Implement and ensure adherence to Monogram’s evidence-based clinical pathways and care model.
  • Review and approve care plans from APPs, RNs, SWs, and PharmDs.
  • Maintain accurate documentation within Salesforce and Athena.

Clinical and Operations Performance – 25%

  • Reduce cost of care and improve clinical outcomes.
  • Lead daily review rounds for high-risk patients.
  • Supervise clinical and operational team members.
  • Oversee daily market operations, including patient engagement, scheduling, and P&L management.
  • Present market performance to leadership.

Patient Care and Treatment – 25%

  • Provide direct and indirect patient care, including diagnosis and treatment.
  • Engage with patients and community providers on treatment plans.
  • Order labs, referrals, and coordinate actions to improve outcomes.

Population Health Management – 20%

  • Guide population health activities, including managing high-risk patients and reducing hospital readmissions.
  • Collaborate with stakeholders to analyze utilization trends and develop strategies.
  • Lead population health discussions in meetings.

Miscellaneous – <5%

  • Participate in on-call activities, including minimum quarterly rotations.
  • Provide coverage for other MPEs as needed.

Position Requirements

  • Remote role within the designated market with occasional in-home visits and travel.
  • Experience applying evidence-based clinical criteria.
  • Background in renal care and geriatrics preferred.
  • Strong management and communication skills.
  • Active, unrestricted medical license in each state within the market.
  • Experience with Medicare Advantage, Medicaid, NCQA, HEDIS, and risk adjustment.
  • MD or DO from an accredited medical school; board certification in relevant specialties.
