Job summary

Legacy Community Health is seeking a dedicated leader to oversee the Ryan White Case Management Team in Houston. You will supervise daily operations, ensure treatment and support services are accessible, trauma-informed, and patient-centered, and guide multidisciplinary collaboration to meet program goals.

Qualifications

  • MSW from a CSWE accredited program is required.
  • Active LMSW licensure as defined by the Texas State Board of Social Worker Examiners is required.
  • Three (3) years of paid work experience in social work post-degree (BSW/MSW) is required.
  • Two (2) years of supervisor level experience including evaluating and managing performance.
  • Experience with the Ryan White program and/or HIV treatment or prevention services preferred.
  • Experience managing programs with direct reports and grant-funded activities.
  • Leadership experience at a manager level is required.
  • Familiarity with ethics and cultural humility is expected.
  • Excellent communication and interpersonal skills across diverse stakeholders.
  • Strong MS Office and EHRS competency is essential.

Responsibilities

  • Provide direct supervision and manage daily activities of case management staff.
  • Oversee daily operations of Ryan White Case Management services and ensure compliance with requirements.
  • Interpret and implement program policies aligned with organizational values.
  • Monitor performance metrics linked to patient outcomes and drive improvements.
  • Supervise recruitment, training, and evaluations of licensed and unlicensed staff.
  • Promote team competency in HIV care and trauma-informed practices.
  • Support professional development and foster a collaborative environment.
  • Ensure comprehensive care plans consider diverse patient needs.
  • Monitor service delivery for access, efficiency, and quality of care.
  • Provide guidance on complex cases with adherence to documentation standards.
  • Lead quality management efforts, including audits and data review.
  • Maintain accurate documentation and reporting; analyze data for trends.
  • Collaborate with teams and partners to ensure coordinated care.
  • Support billing and budget processes aligned with funding requirements.

Key Responsibilities

Minimum Qualifications
  • MSW from a CSWE accredited institution is required
  • Active Licensed Master Social Worker (LMSW) licensure as defined by the Texas State Board of Social Worker Examiners is required
  • Three (3) years of paid work experience, post-degree (BSW/MSW), in the field of social work is required
  • Two (2) years of supervisor level experience, including evaluating and managing performance, supporting process changes in the organization, and coaching, training, or leading a team
  • Previous experience with the Ryan White program and/or individuals seeking HIV treatment or prevention services
  • Previous experience managing practice operations or programs, including supervising direct reports
  • Previous leadership experience at a manager level
  • Previous experience with program management
  • Previous experience in quality or process improvement
  • Previous experience in HIV testing and prevention activities
  • Previous experience with grant-funded activities and/or grants management
  • Previous experience supervising direct reports across multiple locations
  • Knowledge and adherence to the National Association of Social Work Code of Ethics
  • Excellent communication, interpersonal, and customer service skills. Ability to effectively communicate with diverse patients, their family members, medical professionals, and a variety of third-party stakeholders
  • Strong computer skills (MS Office). Demonstrated competency with an EHRS system
  • Proven knowledge of landscape of public benefits and community resources and how to facilitate individuals’ access to those resources
