Job summary

Legacy Community Health is hiring a Bilingual Care Team Assistant to support patient communication and care coordination in a fast-paced clinic environment.

The role requires Spanish fluency, a Medical Assistant certification or equivalent, and experience in an ambulatory setting. Strong communication, organization, and EHRS skills are essential for delivering compassionate patient-centered care.

Qualifications

  • High-school level education or equivalency.
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in inpatient or outpatient setting.
  • Must maintain BLS/CPR.
  • Spanish fluency.
  • Demonstrated competency with an EHRS system.
  • Excellent communication skills.
  • Excellent customer service skills.
  • Ability to multi task in a fast paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
  • Must be able to manage multiple deadline and decision making responsibilities.

Responsibilities

  • Patient Communication: address patient phone calls and route clinical inquiries to providers, return calls and communicate results as directed.
  • Pre-Visit Planning: review patient schedules daily, ensure lab results and consultation reports are available, assist with interpretation services and daily huddles.
  • Care Coordination: manage transitions of care, support access to ancillary services, maintain Care Coordination Desktop.
  • Medication Management: assist with medication refills and prior-authorizations.
  • Forms: initiate and complete forms for review and final signature by the provider, address urgent requests.
  • Referrals: complete referrals, deliver clinical summaries for consultants, ensure documents are in patient records.
  • Safety and Compliance: promote safe operations and adhere to training and equipment protocols.

Bilingual Care Team Assistant - Job Overview

Key Responsibilities

Minimum Qualifications
  • High-school level education or equivalency
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting

  • At least 2 years of experience in an ambulatory clinical setting preferred
  • Spanish fluency
