• Current RN licensure in the State of Vermont (for RN applicants) and hospital experience.

Job description

This role is responsible for assuring the receipt of high quality, cost-efficient care for all patients who seek services at Mt. AscutneyHospital; the oversight of social work, case management,and discharge planning; developing expected patient outcomes, defining appropriate resource utilization and working collaboratively with providers to set goals; assuring the department’s work ahospital compliance is in line with federal and state regulations, reimbursement guidelines as well as the policies and procedures of Mt. AscutneyHospital.

Qualifications

Graduate of an accredited school of nursing or completion of a Master’s Degree Program in Social Work.

Licensure & Certifications

Current RN licensure in the State of Vermont (for RN applicants). Minimum of 5–7 years ofexperience in the hospital setting as a case manager and a member of an interdisciplinary team.
