Assisting Hands Home Care Cave Creek is seeking an experienced, results-driven Community Relations Manager to grow our agency by developing professional referral relationships and consistently generating qualified referrals.

This is not a traditional marketing or event-coordination position. It is a relationship-based, consultative sales position for someone who knows how to gain access to decision-makers, identify needs, present solutions, develop long-term professional relationships, and confidently ask for the business.

The successful candidate will inherit an established book of referral-source relationships while also being responsible for expanding our presence throughout the northeast Phoenix metropolitan area.

We are especially interested in proven healthcare sales professionals who are ready for an opportunity where earning potential is not capped and successful referrals can generate recurring commissions on the services we provide to referred clients during the clients first 12 months of service.

What You Will Do

The Community Relations Manager is responsible for generating qualified referrals by developing and maintaining relationships with organizations and professionals who serve seniors and adults who may need assistance at home.

Primary referral sources include:
  • Hospitals
  • Skilled nursing facilities
  • Inpatient and outpatient rehabilitation facilities
  • Independent living communities
  • Assisted living communities
  • Memory care communities
  • Home health and healthcare organizations

We also expect our Community Relations Manager to identify and develop non-traditional referral sources, including surgery centers, financial advisors, elder law attorneys, fiduciaries, and other professionals whose clients may benefit from home care.

Developing relationships with organizations and professionals serving veterans and their families is another important opportunity within the territory.

Responsibilities include:
  • Build and maintain productive relationships with referral-source decision-makers.
  • Conduct regular face-to-face visits with existing and prospective referral sources.
  • Identify problems and needs and use a solutions-oriented sales approach to demonstrate how Assisting Hands can help.
  • Confidently and professionally ask referral sources for the opportunity to serve their clients, patients, residents, and families.
  • Maintain and expand an existing book of strong referral-source relationships.
  • Prospect for and develop new traditional and non-traditional referral sources.
  • Conduct in-services and community education programs.
  • Maintain accurate referral-source, activity, and relationship information in the company’s CRM.
  • Consistently meet established activity and referral-production standards.
  • Participate in weekly sales, planning, and performance-review meetings.
  • Work collaboratively with operations to understand our service capabilities so that referral partners receive accurate information and dependable service.
  • Perform other duties as assigned by the manager.
Performance Expectations

This is a results-driven position with clearly defined expectations and a structured ramp-up period.

Minimum qualified-referral expectations are:
  • After 3 months: 2 qualified referrals per week
  • After 6 months: 4 qualified referrals per week
  • After 9 months: 5 qualified referrals per week
Activity expectations include:
  • A minimum of 30 referral-source visits per week
  • A minimum of 2 in-services or community education seminars per month
  • Achievement of established weekly CRM activity-point requirements
  • Consistent development of new referral-source relationships
  • Regular face-to-face interaction with referral-source decision-makers

These are minimum performance standards, not limits on what a successful Community Relations Manager can achieve.

The Person We’re Looking For

Our ideal candidate is an experienced sales professional who enjoys building relationships but understands that relationships must ultimately produce results.

You should be comfortable walking into an organization, gaining access to the right decision-maker, asking thoughtful questions, identifying problems, presenting solutions, handling objections, following up consistently, and asking for the business.

Experience Requirements

Strong preference will be given to candidates with:

3+ years of successful home care, home health, healthcare, or medical sales experience

  • Demonstrated success developing professional referral relationships
  • Experience gaining access to discharge planners, social workers, case managers, and other professionals who influence a family’s decision to obtain home care
  • Proven consultative or solutions-oriented selling skills
  • A documented history of meeting or exceeding sales or referral-production goals
  • Experience using mobile/app-based CRM tools to manage referral partners, prospects, and sales activity

Exceptional candidates from outside home care, home health, or healthcare may be considered with at least 5 years of successful relationship-based sales experience and a demonstrated record of building long-term professional relationships and producing measurable results.

Compensation and Earning Opportunity

This is a performance-driven sales position with uncapped earning potential. The compensation plan includes a structured ramp-up period designed to provide greater income stability while the Community Relations Manager learns the territory, strengthens existing relationships, and develops a pipeline of new referral business.

The compensation progression is:

  • Months 1–3 – Base + Commission: The Community Relations Manager receives an initial base compensation plus commission while becoming established in the position and developing the referral pipeline.
  • Months 4–6 – Reduced Base + Commission: Beginning in the fourth month, base compensation is reduced as the Community Relations Manager assumes greater responsibility for generating commission-producing business.
  • After 6 Months – Compensation Choice: Beginning after the first six months, the Community Relations Manager may choose between:
    • Base + Commission: Continue receiving base compensation and generally earn a commission equal to 3% of gross revenue derived from referred clients during each client’s first 12 months of service; or
    • No Base/Higher Commission: Elect a no-base compensation structure and generally earn 4% of gross revenue derived from new clients referred after the transition during each client’s first 12 months of service.

Clients referred while the Community Relations Manager is participating in the base-plus-commission structure remain at the commission rate applicable when those clients were referred.

There is no cap on earning potential.

As the Community Relations Manager generates more qualified referrals and those referrals become clients, the opportunity to build a larger portfolio of commission-producing business increases.

This compensation structure is designed for a successful relationship-based salesperson who wants to be rewarded directly for developing productive referral relationships and contributing to the growth of the agency.

The specific terms, eligibility requirements, commission calculations, payment timing, and other conditions governing compensation are set forth in the Company's written compensation plan.

Territory and Schedule

The primary territory is the northeast Phoenix metropolitan area, including:

North Glendale North Phoenix Anthem New River Cave Creek Carefree North Scottsdale Paradise Valley

The typical schedule is Monday through Friday, approximately 8:00 a.m. to 5:00 p.m. Because relationship development sometimes occurs outside normal business hours, occasional evening and weekend events may be required.

This is primarily a field-based position. The Community Relations Manager is expected to work from the office approximately one day per week for sales meetings, weekly wrap-up, performance review, and planning sessions with the manager.

Employment Requirements

Employment is contingent upon successful completion of the following requirements after a conditional offer of employment has been made:

  • Satisfactory professional reference checks, including references from prior employers relevant to the candidate’s sales experience
  • Successful completion of a national background check
  • Successful completion of required drug screening
  • Verification of a valid driver’s license and ability to meet the travel requirements of the position

Final candidates will participate in a practical sales role-play designed to demonstrate consultative selling, relationship development, objection handling, and the ability to ask for the business.
