Western CT Home Care Inc is seeking a Home Care Intake Manager to lead referral management, ensure smooth transitions from hospital to home health, and supervise the intake team. The role requires CT RN licensure, BLS, and several years of clinical/home health experience.
The position emphasizes education program development, collaboration with hospital care teams, and maintaining high-quality care metrics. Full-time hours with in-field supervision are expected.

Summary:
Provides direction and organizational management of the home care intake department including referral management, referral portals, physician group referrals, SNF/ALF discharge planning meetings, and the transition of patients from the acute setting to home. Responsible for the educational, orientation, and competency programs of the agency. Per diem nursing staff report directly to this position for recruitment, hiring, and performance evaluation. Supports the other nursing clinical managers in the department daily operations as needed.
Education: Bachelor’s degree in nursing and a minimum of three (3) years of clinical experience in nursing, at least one (1) of which was in home health or community health program which included care of the sick at home, or an Associate Degree in Nursing with 5+ years of experience, one of which was in home health or community health that included care of the sick, and completion of 6 credits received in Community Health Nursing Theory, or 6 credits in Health Care Management from an accredited college or university program or school of nursing. The 6-credit course must be completed within 18 months of hire.
Credentials: Active State of Connecticut RN license; current BLS certification

Skills: Proficient in the use of electronic medical record systems; experience with Microsoft Word, PowerPoint, and basic Excel spreadsheets; excellent organizational and professional communication skills; able to work with teams across departments both internal in the agency and within the health system.
Essential: Must have an active driver's license and automobile in good working condition.
