Job summary

Wilmington Health is seeking a Diagnostic Breast Imaging Patient Navigator to guide patients through the imaging process, coordinating mammography, ultrasound, and related procedures while serving as a primary contact for patients needing additional imaging.

You will track follow-ups, coordinate orders and reports, and communicate with providers and radiology staff to ensure timely, compassionate patient care in a multi-specialty practice.

Qualifications

  • High School Diploma or equivalent required.
  • Healthcare scheduling or patient coordination experience preferred.
  • Strong communication, organizational and time-management skills.
  • Ability to document accurately and manage multiple patients.
  • Professional and compassionate interaction with patients and staff.
  • EHR and scheduling experience preferred.

Responsibilities

  • Coordinate and guide patients through the diagnostic breast imaging process.
  • Serve as a primary contact for patients requiring additional or diagnostic imaging.
  • Schedule diagnostic mammography, breast ultrasound, and related procedures.
  • Provide clear, compassionate information on next steps in the imaging pathway.
  • Track patients requiring additional imaging and assist with timely follow-up.
  • Coordinate orders, prior imaging, reports, and other information for appointments.
  • Communicate with referring providers, radiologists, technologists, and staff to coordinate care.
  • Assist patients with scheduling, records, orders, and other barriers to care.
  • Document patient communication and scheduling activity in the system.
  • Escalate clinical questions or unresolved follow-up to the appropriate team.
  • Maintain patient confidentiality in line with HIPAA and regulatory requirements.

Purpose:
  • Coordinate and guide patients through the diagnostic breast imaging process, serving as a liaison between patients, mammography staff, radiologists, referring providers, and scheduling teams to facilitate timely care, clear communication, and appropriate follow-up.
  • Serve as a primary point of contact for patients requiring additional or diagnostic breast imaging.
  • Coordinate and schedule diagnostic mammography, breast ultrasound, and recommended breast procedures as appropriate.
  • Guide patients through the diagnostic imaging process, providing clear and compassionate information regarding next steps.
  • Track patients requiring additional imaging or procedures and assist with timely completion of recommended follow-up.
  • Coordinate necessary orders, prior imaging, reports, and other information needed for diagnostic appointments.
  • Communicate with referring providers, radiologists, mammography technologists, and other team members to coordinate patient care.
  • Assist patients with resolving scheduling, records, orders, and other barriers that may delay diagnostic care.
  • Document patient communication, scheduling activity, follow-up attempts, and outcomes in the appropriate systems.
  • Escalate clinical questions, unresolved follow-up, or other concerns to the appropriate clinical team member.
  • Maintain patient confidentiality and follow established departmental, HIPAA, and regulatory requirements.
Other Duties:
  • As assigned by Radiology Manager and Mammography Leadership
QUALIFICATIONS
Required
  • High School Diploma or equivalent
  • Previous healthcare, medical scheduling, patient coordination, medical assisting, or related patient-facing experience preferred
  • Strong communication, organizational, and time-management skills
  • Ability to manage multiple patients and follow-up needs while maintaining accurate documentation
  • Ability to communicate professionally and compassionately with patients, providers, and clinical staff
  • EHR and scheduling experience preferred
Preferred:
  • Previous experience in mammography, breast imaging, women’s health, radiology, oncology, or patient navigation
  • Knowledge of medical terminology and diagnostic breast imaging workflows
  • Experience with PACS/EHR systems and patient tracking or coordination of follow-up care
