You’ll collect health info, relay Provider plans, and document in the medical record with accuracy. Requirements include a high school diploma or GED, MA training or 3 years MA experience, NH LPN license, and BLS/CPR certification.

Qualifications

• High school diploma or GED required.
• MA program completion or 3 years MA experience preferred.
• LPN license NH or compact state required.
• BLS/CPR certification is required.
• Nursing diploma preferred.

Responsibilities

• Participate in patient care in person, by phone, and via telehealth under supervision.
• Administer medications, treatments, immunizations, point-of-care testing, and specimen handling as delegated.
• Collect and screen patient history and health information and relay findings to Providers.
• Communicate care plans and advice to patients per Provider guidance.
• Document care activities in the medical record accurately.
