The role includes administering prescribed medications and treatments, collecting patient history, communicating plans of care, documenting activities in the medical record, and supporting telehealth and in-person visits.

Qualifications

• Provide nursing care under supervision of RNs/providers.
• Administer medications and immunizations within scope.
• Collects and screens patient history and communicates findings.
• Document care activities accurately in the medical record.

Responsibilities

• Participate in patient care in person, via phone screening and telehealth.
• Perform delegated nursing tasks within scope of practice.
• Collects patient information and communicates plan of care to providers.
• Document test results and care plans in the medical record.
