Job summary

Lexington Medical Center is seeking a licensed Physical Therapist to join our clinical team. You will provide patient evaluations, develop treatment plans, and deliver evidence-based therapy to diverse patient populations.

Responsibilities include maintaining accurate documentation, collaborating with physicians and staff, supervising support personnel, and promoting high-quality care. Relocation assistance of up to $7,500 is available for qualified candidates.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Provides patient care assessment and treatment.
• Maintains accurate documentation including evaluations, progress notes, and discharge notes per policy.
• Ensures comprehensive, functional evaluations and treatments with realistic goals and sound professional judgment.
• Supervises support staff assisting with patient care and provides direction and feedback to team.
• Promotes professional development, communicates with patients and staff, and follows department policies to improve quality of care.
