Crusader Community Health in Rockford, Illinois is seeking a Living with HIV Registered Nurse Case Manager. This role is responsible for coordinating medical interventions through specialty clinics and residential site visits, promoting optimal health, and applying a holistic approach to client services.

The Living with HIV Registered Nurse Case Manager is responsible for coordination of medical interventions through specialty clinics and community/residential site visits, and promotion of optimal health through monitoring and interventions. The RN Case Manager applies a holistic approach to the coordination of client services.

The salary range for this position is $59,507-$90,599. Actual compensation will be determined based on factors such as relevant experience, qualifications, and internal equity.
