Job summary

The Springs Post-Acute in Norwalk, CA is seeking a Licensed Vocational Nurse (LVN) to assist RNs in delivering high-quality care in a 99-bed skilled nursing facility. You will participate in medication administration, assessments, and care coordination in a team-driven, compassionate environment.

Duties include monitoring vital signs, wound care, and contributing to care plans while ensuring compliance with policies and standards. Competitive pay and comprehensive benefits are offered.

Qualifications

• Must be a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state.
• Must be able to read, write, speak, and understand English.
• At least 1 year of Treatment Nurse experience.
• Ability to work harmoniously with other personnel.
• Willingness to learn and adapt nursing practices.

Responsibilities

• Conducts initial and ongoing assessments of residents' health status.
• Monitors vital signs, administers medications, and observes changes in condition.
• Notifies RN or healthcare provider of significant changes.
• Administers medications as prescribed and ensures correct dosage and route.
• Monitors residents’ responses to medications and documents findings.
• Performs basic wound care and other treatments as ordered.
• Coordinates care with RNs, nurse managers, CNAs, and other staff.
• Participates in care planning meetings and helps develop resident care plans.
